Question: my daughter is 30 years old and has had two treatments with Acutane for cystic acne in the past, and continues to have terrible cysts and breakouts. She has been told she cannot take another course of the Acutane. What can be done or recommended for her, she does have some scarring, but, essentially wants to get rid of the constant break outs, as they are painful.

Thanks for your e mail. Active cystic acne is something best cared for by a Dermatologist and if she has had 2 courses of acutane, further treatment could result in worsening of the scars, at least in the first year. I'd recommend continued care by her dermatologist to try to keep the acne under some degree of control. I hope this helps.

Question: Dr. Edwards, If i were to have a breast augmentation done, how long should I wait before I try to have another baby? Is something like a month after the procedure too soon? Thanks

I do not recommend you becoming pregnant that soon after a breast augmentation for a variety of reasons. It takes weeks to months to see the final results of a breast augmentation and it is possible that skin stretching that can occur with pregnancy, especially so soon after augmentation, could make your result not the best. Subject: Breast lift, augmentation, or both?

Question: Hello Dr. Edwards,
I am wondering if I would be a candidate for a breast lift or augmentation? My breasts have gone from a B cup to a VERY small A if not AA since having four children. I was wondering if a lift would do anything for me at all. I am young, 5"6, and 128 lbs. and I am just interested in having my normal breasts back. If a lift would not bring my breast tissue back up where it belongs, I was considering implants. I would like a very natural looking C cup. I have ZERO breast volume left. Any info would be great, I have no way to call your office so email is preferred. Thank you!

Whether someone needs a breast lift with their augmentation depends on the laxity of your breast tissue and the position of your nipple on your breasts. Basically if you were to place a pencil under the fold of your breast and then look at that level, if your nipple sits at or lower than the nipple, you most likely will need an accompanying breast lift. i have some concerns about silicone implants, i have been reading on these type of implants and was wondering what type of silicone do you use. Soft filled semi liquid or firm filled cohesive gel? Also do you use the "gummy bear" type of implant that i have been reading about which is suppose to be more safe?

The only currently FDA-approved implants are cohesive gel implants that were studied under FDA protocols for 14 years before they were approved as safe. The "gummy bear" implants are not yet approved and are firmer than the ones being used now. I think the FDA would not have approved them if they did not feel they were safe but I tell my patients if they still have concerns about them, they should probably use a saline-filled implant instead. Question: I had submuscular augmentation done a year ago, and one breast had a severe crease from the center of the cleavage about 3/4 of the way across the breast when the peck was flexed. My doctor referred to it as an "activation deformity" and said it's not uncommon, but most of my friends have implants and none have anything like that... Have you experienced this in your patients, and how did you address it? I am looking at obtaining 2nd opinions, but wanted to know your thoughts on the issue before I start scheduling anything.

I think this is simply a way to describe a change in the shape of your breast which occurs as a result of contraction of your pectoralis (chest) muscle. I have seen on a number of occasions that this may be able to be improved with a release referred to as a dual plane release but that only way to be able to properly tell you would be to see you in consultation. It will be very helpful at that time to have any information on your surgery that you have otherwise records can be requested from your surgeon. To my knowledge I have not performed a breast augmentation on a patient with MTHFR. I performed a search and there are some associations with certain medical conditions but I did not find anything that noted a contraindication to elective surgery. Question: I had liposelection less than i year ago ( april will be a yr.) At first it looked good as i was healing. I also had to keep going back to the doctor once a week because i kept filling up with fluid (seroma) the problem now is that i am getting full in the same area's that were treated. (abs and flanks). I have only gained maybe 5 lbs . but it looks like alot more in my mid section. I was told that if you gain weight after lipo it would be in other areas not treated. I excercise faithfully 4-5 times a week and try to eat healthy. I cannot seem to lose weight either. I wonder if it is a metabolism problem or hormones. maybe insulin resistant. I now need a tummy tuck the lipo left loose skin. would you recomend i see my doctor for tests to see why i cant lose weight and continue gaining belly fat? What is the cost for a tummy tuck ?

Question: My husband & I are in the process of moving to Nevada and I am researching my options to have breast augmentaion done. It is something I have wanted done nearly all of my life. My question is, what kind of risk is there if I am a smoker and I do take medication for hypertention, my husband & I have been argueing about this for sometiome now. I told him surely he does'nt think that I am the only woman in the world that smokes and takes medication that wants to have this procedure done.......I realize it would be best if I not smoke, but the fact is I am a smoker....

Dear Flat in KS,
There are many factors to consider in a purely elective surgery such as breast augmentation. I ask all of my patients to not smoke or be exposed to tobacco products for AT LEAST 2 weeks before and after surgery. Smoking or being around smoke increases your risk of infection and other post-operative complications. If you take medication for your blood pressure you would need to have additional pre-operative testing (EKG, extra lab tests, and probably medical clearance from your primary care Dr.). Before your surgery you would need to have an initial consultation with me to review thoroughly your medical history before a plan can be made.
