I really hope you can help me with my situation. I'm a male, 42yrs. old, and overweight. My problem is sort of hard to talk about or describe, but I guess the best name for it is a "buried penis or trapped penis". I'm sure you probably have heard of it.
The area around the base of my penis (abdominal fat pad area) has made it so it retracted. It's getting very painful. I would need that area worked on I guess. I hope you know what I'm talking about. And I hope that would be a type of surgery you perform.

Question: A couple of questions:
I currently live in a smaller city (and I have not heard great things about the surgeons here) and I am wondering if you perform surgery on people who do not live in Vegas? I assume if I am willing to travel to my post-op appointments, you would perform the surgery?
My next question is regarding when a woman gets older (as in 65, 70, 80) I assume that one would remove the implants. What will be the result at that point? I have only found one piece of information on the internet about a feeling of having a "tennis ball"on your chest. I assume that the scar tissue that forms around the implant would be felt? Is there any information out there about what it is like after you remove your implants?

I do commonly see patients from out of Las Vegas. This can be accomplished with a safe outcome with proper planning. I also appreciate you knowing the importance of follow-up which I feel is crucial to a good result. You don't have to automatically have your implants removed when you are older. They look of your breasts if you do remove your implants depends on the changes that have occurred in your breast tissues. Subject: Implants gone wrong

Question: I know it will be hard for you to answer this without a consultation, but I would to get an idea if I could be 'fixed' or not. About 3 years ago I had surgery to correct severe asymmetry. I had an implant placed in my right breast and a lift for my larger C-cup left breast. The augmentation turned out ok, except that the implant wasn't large enough to match the left side. But the breast that was lifted was lifted way too high and my nipples are no longer in line (they were to start with). The nipple is now so high that it tends to show over the tops of some of my shirts. The incision also tore back open after the stitches were removed and I had a hole the size of a 50-cent piece that has now healed over into a horrible scar. Based on this information do you think this can be repaired? If so I'd love to meet with you.

Breast asymmetry is quite common but some are more noticeable than others like sounds as though yours was. I think there are things we can do to improve your nipple position and scars but as you mentioned, I would need to see you before I could give specific possible solutions. Question: You are amazing to answer all the questions - thank you so much for informative answers and not uninformed hear-say!! I really would like a BA, but my long term health is more important to me, so the following questions if you can help- 1.Is there a correlation between BA and auto immune illnesses, lupus, etc or is it just a very small percentage? 2. What % of patients get capsular contract - big or small risk? 3.Will almost half of silicon implants rupture after 10 or more years? So, must I be prepared to get a new set of implants in my life (I am 35) if I get a BA now? 4. If gummy-bears get approved by the FDA, do you think it will be within a year, or must a 5 or more year study first be done? I'd like to wait for that. And I assume you will also implant gummy bears once they are approved? 5. If I do a BA, must I go for a MRI every 2 or 3 years or is it not neccesary? THANK YOU SO MUCH!!

The best place to start would be to see you in consultation, go over your
history and then examine you. At that point we could review all of your
options and we would provide you with the costs involved. I do perform
implant removal surgery if that is what we decide is best for you but there
may be other options. It will be helpful at your appointment if you have
any information at all about your surgery to include implant information.

The apron of lower abdominal skin is referred to as a pannus and is treated in general with a panniculectomy. Every patient needs to be evaluated individually but I do not offer abdominoplasty to women who have a body mass index of greater than 30 due to the increased risk of complications. If your apron is as a result of weight loss, a panniculectomy may be your best option but the only way to tell is by examination and reviewing your medical history. Botox is the most common non-surgical procedure we do. We have a 40 unit minimum and with a cost of $10/unit, that is $400. Question: Hi Doctor. I had a breast lift with augmentation. First few months they looked great, but now almost at 6 months they have dropped considerably with a lot of rippling on one of them. My doctor said he'll probably have to close the pockets but that I have to wait an additional 3 months to do that. Is this common with breast implants and how invasive is it to have this other procedure done? Why didn't he make the pocket smaller to begin with? Also, why do I need to wait all this time. I am so upset about this. Any insight into this would be greatly appreciated. Thank you.

Dear Impatient,
It is much easier to advise you if I were to see you in consultation but your breasts do go through changes after surgery and it is difficult to completely predict how much laxity can occur after breast surgery. It depends on your tissues, the size and type of implant used, the surgery to some extent, etc. I encourage you to follow up with the surgeon who you trusted in the first place to perform your surgery. I do perform a lot of revision breast surgery but you are still in your first year post-op. The reason that you would need to wait is for the capsule or scar that forms around the implant to mature so that if the pocket needs to be made smaller, there is a strong tissue to sew to. Question: I am 40 years old and I have 3 children. Add to that I have lost almost 60 pounds. Needless to say, I am not happy with the way my breasts look. There is no question that if you plan on decreasing your weight by 50 additional pounds that you should wait on any elective surgery. You wouldn't want to have the surgery and then have recurrence of skin laxity after losing the weight. All things considered, you can have the 2 surgeries at the same time. As appealing as the idea may be I think it would be ill-advised to do this. We need to allow your body to return to a pre-pregnancy state to let you get the best possible result from a tummy tuck. Question: I am 35 and very petite. I suffer from a slight case of "sucken chest" (sorry, I do not know the medical term), and after breastfeeding 2 babies, I need new breasts. This condition is slight, not severe, and actually was helpful when I was younger to make my cleavage appear larger than it really was. I am now a saggy A cup. I am terrified that they will not be placed right on my chest or will be too big. All I want is a full B or smaller C, and would like to know if you have ever worked on someone with this condition. And do you encourage strongly that your patients go always go BIGGER??

Thanks for your question. It is common that women have rib cage asymmetry (prominence or sunken) and that the way your breast augmentation is done is a little different. I treat each women individually and the size we chose depends on your anatomy and what you are hoping to achieve. Not all women require large implants. Question: I wanted to make an appt. with you to have some type of filler done. I just turned 37 and I'm thin with a petite frame so unfortunately while my body is good, my face has begun to look hollow around the cheeks. My preference for the use of fillers is a conservative approach and I do not typically get an over done look. The way I do it is to have you look intermittently as I am performing the procedure to allow you the input that you should have. I have a number of patients who tell me that the severity and frequency of their migraines does diminish with Botox treatment. To my understanding this is not an FDA-approved use of the product and so, like many uses of Botox, it is considered off label. The use of Botox in the state of Nevada is such that only one patient can be treated with one vial and therefore my office policy is that there must be a 40 unit minimum purchase of a 50 unit vial. The basic costs is $12/unit, therefore $480. My office has special offerings on Botox on occasion so it would be best to speak with my office staff to get the details. Every woman's body is different in what changes result from either a breast augmentation with or without a lift as well as with abdominoplasty. Therefore I couldn't venture to guess what your post-op measurements would be, most certainly without meeting with you and examining your tummy. With regards to your breast, depending on the degree of skin and tissue laxity over your implants, there are different lifts that can be performed from around the areola all the way to a full lift. Based on your height and weight your Body Mass Index is almost 33kg/m2. This is too high for elective liposuction surgery in my care. Have you been back to your gastric surgeon to see if the pouch has stretched or if something can be done. Liposuction is not a means of weight loss so that is where I would start.

The pain you are having could be due to the injured nerves or if your breast is becoming more firm, it may be due to a capsular contracture. The things you can do are wearing the best supportive bra that you can that minimizes jostling. You can also try a proper dose of Motrin for the pain but make sure and stay well hydrated, especially where you are. It is most likely the capsule that is uncomfortable but it is hard to tell. Thanks for your question. Large breasts can many times be difficult, especially in a woman who stands 5 foot tall with 38DD breasts. Other than wearing a good supportive bra, there are not many options for changing breast size and therefore the weight of them on your back and shoulders. You did not mention your weight. If you are a little overweight, loss of weight by a good diet and exercise regiment may also decrease the volume of your breasts.

Using your height and weight that allows us to calculate your BMI (Body Mass Index). Your's calculates to 37kg/m2. This may not mean anything to you but it allows me to guesstimate if you are at a higher risk for complications. I prefer not to perform an abdominoplasty on a patient who's BMI is greater than 30. For your BMI to be at 30, you would need to decrease your weight to approximately 210#. There are many other factors to consider but this is most certainly an important one to start with. I don't what your height is but with your weight being 260#, you would not be in the best position to have a tummy tuck (abdominoplasty). After you have had the issues of your GYN problems cleared or corrected, you would do best with a good weight loss regiment. Active cystic acne is something best cared for by a Dermatologist and if she has had 2 courses of acutane, further treatment could result in worsening of the scars, at least in the first year. Whether someone needs a breast lift with their augmentation depends on the laxity of your breast tissue and the position of your nipple on your breasts. Basically if you were to place a pencil under the fold of your breast and then look at that level, if your nipple sits at or lower than the nipple, you most likely will need an accompanying breast lift. The only currently FDA-approved implants are cohesive gel implants that were studied under FDA protocols for 14 years before they were approved as safe. The "gummy bear" implants are not yet approved and are firmer than the ones being used now. I think the FDA would not have approved them if they did not feel they were safe but I tell my patients if they still have concerns about them, they should probably use a saline-filled implant instead. I think this is simply a way to describe a change in the shape of your breast which occurs as a result of contraction of your pectoralis (chest) muscle. I have seen on a number of occasions that this may be able to be improved with a release referred to as a dual plane release but that only way to be able to properly tell you would be to see you in consultation. It will be very helpful at that time to have any information on your surgery that you have otherwise records can be requested from your surgeon. I performed a search and there are some associations with certain medical conditions but I did not find anything that noted a contraindication to elective surgery. Question: I had liposelection less than i year ago ( april will be a yr.) At first it looked good as i was healing. There are many factors to consider in a purely elective surgery such as breast augmentation. I ask all of my patients to not smoke or be exposed to tobacco products for AT LEAST 2 weeks before and after surgery. Smoking or being around smoke increases your risk of infection and other post-operative complications. If you take medication for your blood pressure you would need to have additional pre-operative testing (EKG, extra lab tests, and probably medical clearance from your primary care Dr.). Before your surgery you would need to have an initial consultation with me to review thoroughly your medical history before a plan can be made. I am a 44 year old women. when I was 21 I had cancer and had 3 abdominal surgeries in 7 months. Eight weeks after that I had 8 weeks of radiation therapy. This was done at Sloane-Kettering in NYC. I am 5'4 116lbs and the skin on my lower stomach does not tighten with excercise. I work out 3 to 4 times a weeks and eat very good and I don't smoke. Thanks for your question. I do not have specific impact studies performed on breast implants readiliy available and a search of the web failed to yield any good data. I know that current breast implants are designed to withstand 25 times the force of a standard mammogram which is significant. I have performed breast augmentation on many female atheletes who participate in different sports (running, tennis, golf, bikin, weight-lifting) Although I did not perform her surgery I have a female friend who partcipates in contact sports. I'm not sure what kind of impact you are expecting to experience after surgery but I would be happy to meet with you to discuss what your goals are and what procedure would best help you meet those goals. Thanks for your question. It sounds from your derciption like you have lost breast volume after breast-feeding your children but you have laxity of the breast skin. This is a common question and many times a breast augmentation alone is the wrong choice because of the extra skin. It sounds like you would be a possible candidate for a breast augmentation combined with a breast lift but the only way to tell if this would be your best option would be to meet with you, examine you, and then review your options. I had to do a Google search to learn a little about MTHFR (Methylese-Terta-Hydro-Folate-Reductase). It sounds as though it is a very common mutation, one source I found said that 44% of the population is heterzygous. By my quick research it is a dosorder that can make you more prone to forming a blood clot and the concern with any surgery would be a blood clot forming in your deeper veins formin a Deep Vein Thrombosis (DVT). I believe that you could still have breast augmentation surgery but I would want you to be off the baby ASA for 2 weeks before surgery to decrease the risks of bleeding and bruising due to the effect on your platelets. I would need some guidance from your hematologist for how best to handle your per-operative care. Not everyone is a candidate and you must be aware that it typically involves a scar that travels the length of your upper arm and sometimes onto your chest. Question: I currently have saline implants, over the muscle, have had them for 6 years. I do have some rippling as I am lean and muscular. I would like to switch to silicone and wonder if submuscular would be better? I have a very physical job and rely heavily on upper body strength (why I chose overs). I am also tested on push-ups, pull-ups, etc, yearly so loss of strenght would be noted. Is it contraindicated to do heavy upper body lifting with unders? It sounds as though you should stay with your implants above the muscle. There are a number of well-conducted studies that show no decrease in strength with submuscular implants but since you have a pocket above your muscle now that would have to be closed with a site change surgery, and you are very physically active, I would tell you that there may be some motion of your breasts with weight lifting, etc. The best way for me to be able to recommend a course of treatment would be to meet with you, examine you and then go over options. Thanks for your questions. You are young to be considering an abdominoplasty, especially if you may be considering more children. It may be that liposuction could be a help but I would need to examine you to tell you what would be best. Thanks for your question. There is no substitue for proper diet and exercise to trim and tone your body. Liposuction is not a means of weight loss but is useful in contouring. The results you get depend upon the tone and health of the skin overlying the areas to be treated. I'm not sure why someone would tell you it would return in double the mass but if you had surgery and then didn't care for yurself in terms of sensible diet and exercise, you can most certainly gain weight. Most certainly self breast exams and mammograms are effective and necessary after breast augmentation. I believe it helps if the implant is under the muscle but you always need to let the mammographer know that you have implants. There is some literature to support that breast implants may obscure the imaging of a small area of the breast often times requiring additional views.

The issue of smoking and surgery is an important one. The resone to not smoke for at least 2 weeks before and after surgery is to minimize your risks of post-operative complications such as infection and poor wound healing. The carbon momoxide and nicotine in all tobacco products (to include first and second hand smoke) causes the blood vessels to spasm decreasing the blood flow to the area healing. There is a blood test that can be run to determine nicotine levels and I have cancelled surgery in the patient who continues to smoke only because I want you to have a great result and there is no sense in gambling with your results. All things considered you could undergo a removal and replacement of your saline implants with larger silicone implants with a mini-abdominoplasty. There are limited indications for a mini-abdominoplasty depending on what you anted to correct. I would approach your breast surgery through the same scar under your breast but it would not need to be 4 inches in length. The science of scars, healing and modification is a growing science with breakthroughs coming often. Revising problematic scars is a vexing issue that has yet to be conquered. The options for scars in the skin are, topical treatments, silicone gel sheeting for pressure, pressure treatment, steroid injection and revision. Each has its potential place. I believe you are referring to a scar that is attached to the muscle which can cause tethering and sometimes discomfort. Some of these can be improved by revising the external scar but many times scar massage and time will suffice. I hope this helps. Dr. Edwards

Do you get dermabrasion or microdermabrasion? Microdermabrasion removes a shallow amount of superficial cells and is good in preparation for other skin procedures such as peels or laser treatment. Dermabrasion requires some degree of anesthesia and is typically not done monthly. You sound as though you would benefit from a visit with a medical aesthetcian who can get you started on a good regimine of skin care to possibly include some form of bleaching cream. There are lasers that can help tighten you skin as well to help with the fine lines but your best bet is a proper skin evaluation by someone who really knows what they are doing. Dr. Edwards

I had a baby 4 months ago and my breast size (which was a B cup before the baby) has decreased to about training bra size! I am considering breast augmentation, however, I was diagnosed with ITP in December 2005 after finding out I was pregnant for the second time. My platelet count did not resume to a normal count afterwards and currently hovers at 30,000 or above. I do not take any medication for it (I was previously on Prendisone) anymore, I just have my platelet count checked twice a month. Would you perform this surgery on me with this blood disorder? To have an elective operation we would need your platelet count to be higher. The risk of bleeding would be too high at 30. I also would like you to not have been on prednisone for awhile as well but please do remember to bring that up at consultation in the event we need to give you stress dose steroids. I would need you to see your hematologist before surgery and we would need to decide together what we want you platelets to be and how best to achieve that. It may be by steroids or a platelet transfusion (this is most certaily the second best choice). Your hematologist will also be a better person to discuss the role of electievly removing your spleen as well. I am just drawing on my general surgery days so please realize that your hematologist is the ITP expert. I do not recommend the TUBA (trans-umbilical-breast-augmentation) for any of my patients. If you were to have bleeding, you would end up with an incision on your breast anyway to treat it. Thanks for your e mail. There are a couple of things to consider when you want to have a breast augmentation after having a baby. I ask patients to wait at least 3 months after they have stopped breast-feeding to allow your breasts to return to a pre-pregnancy size. It also allows us to note if there s any laxity in the breast skin and tissue to ba accounted for. Regards lifting, I typically ask patients not to lift greater than 10 pounds for 6 weeks after surgery but that would make it difficult with your new baby. Skin reduction procedures for patients who have lost weight after gastric by-pass are becoming more and more common. ou should be stable at your weight for at least 6 months before undergoing surgery to get a good result. Thighlift scars can be in the crease of the thigh and in some cases run down the inner aspect of the thigh. Abdominoplasty incisions vary as well. An inverted nipple is typically caused by a shartening of the ducts that connect the nipple to the breast tissue therefore the mainstay of treatment is to make a small incision at the base of the nipple and divide those fibers. The downside is that there is a high liklihood that you would not be able to breastfeed in the future and there is a chance of decreased sensation after the procedure. I hope this helps. Thanks for your e mail. Unless an implant is used for reconstruction, they are not typically covered by insurance. Silicone implants are still under what is referred to as an Adjunct Protocol and they have not been released by the FDA for general use. The first important step in caring for acne is to get the active process under control. Your best bet is to see a dermatologist who can evaluate you and prescribe a course of treatment which may include topical or oral antibiotics or even acutane. Once the acne is under control, then you can explore avenues of treating the scars if need be. This will also depend upon a number of factors. I do not personally perform any facial surgery in my practice to include chemical peels, dermabrasion or resurfacing. You would need to locate a board-certified plastic surgeon who does to care for you at that point if it is deemed necessary.
I strongly think any thought of suicide should be discussed with a counselor sooner rather than later. Most patients outgrow their acne as your body changes.

Question: I am a 25 year old female, who does not drink or smoke. On July of 2005 I was diagnosed with SLE. As a result I was found to be in stage 4 out of 5 kidney failure and anemia. Almost a year later I'm in full remission and off predisolone. My endocrinologist tested and found my adrenal glands are responding well and my weight in slowly returing to normal.
However, I'm left with some terrible striae/stretch marks over my stomach and breasts. They range in width and length from 1in maximum wide to 6 inches long maximum. Also there are striae on the top and bottom of my breasts.
I have been figure skating about 12 hours total a week and reducing my diet to 1200 calerioes a day--it's been working however my skin in those areas I fear will always be loose and will never return to the way I remember it. In addition, they can be rather uncomforable if pinched between the band of my jeans.
My husband is an active military officer and supports my desire to seek plastic surgery. However, I have two questions. One, could someone having been diagnosed with Lupus undergo plastic surgery? Two, what procedure is recommended?

Thanks for your question. Every woman is different in terms of which implants help them achieve their augmentation gooals. In my office we have you actually place the implants in a sports bra and view yourself in a mirror to give you a guestimate of what they would look like. I try not to make recommendations re cup size because there is such a difference in bra manufacturers and how they fit patients. Implants only need to be changed if they deflate, if you want yo change to a different size in the future of sometimes if you develope a capsular contracture (tight scar around the implant). If you plan on continuing to lose weight, it is bet to complish that first and then be stable at that weight for at least 3 months. Imagine if you had a tummy tuck and or breast lift and augment, and then you lose 40+ pounds, I'm sure you would have more sagginess possibly causing you to want/need more surgery. When you are certified by the American Board of Surgery, this means your are a general surgeon (gall bladders, hernias, etc.).The only way to know that someone is truely a Plastic Surgeron is if they are certified by the American Board of Plastic Surgery or if they are board elgible. I do not know who your doctor is but they may have completed a plastic surgery residency and have not yet taken their plastic surgery boards (board elgible) or, they may be what is referred to as a cosmetic surgeon. This is a commonly asked question. It really depends on many factors but an implant placed partially below the chest muscle for augmentation has a low risk of interfering with future breast feeding however there cannot be 100% guarantee. Depending upon how much change there was in your breasts after your first pregnancy, you may have minimal change after breast augmentation. It depends on the tightness of your skin, how much breast tissue you have and the size implant you have placed. The bottom line is that you certainly can undergo breast augmentation safely in-between pregnancies. I would be happy to meet with you to discuss your best options.

The 2 issues you have conerns about, rippling and a "stuck on" look are things I try to avoid. Like you mention, going too large is a major contributing factor as well as placing implants above the muscle. I place virtually 99% of the breast implants in the partially sub-muscular position which helps to give a more natural upper breast look and minimizes upper pole rippling. Rippling in the lower outer aspect of the breast has everything to do with the thickness of your tissues and the size of the implant. Breast reduction surgery is one of the most gratifying procedures that a plastic surgeon performs. This is based upon the comments the patients make afterwards. The trade off though is the scars. There are photos of patients on my website of women who have had breast lifts and reductions for you to see. There are different techniques but I would need to examine you in order to see what would work best for you but most all involve repositioning the nipple and areolar complex. I have performed breast augmentation in over 850 patients to date. I have had patients who have experienced complications. I wish I could state that I had a 100% 1-5 year follow up with all of my patients but many move away or don't return after 1-2 years, presumably because they are doing well without problems. The number of patients I have had to treat for capsular contracture that I have performed their initial augmentation is very low, most likely due to the fact that I place virtually 100% of my patient's breast implants under the chest muscle. Liposuction in the upper abdomen is not an uncommon request after abdominoplasty. This is because it is difficult to do this at the same time as the tummy tuck due to possible compromise of the abdominoplasty blood supply and subsequent wound healing problems. Realize the liposuction does not tighten the abdominal wall, but will remove only some of the fat under the skin. To remove breast tissue and place a breast implant without a lift is rarely possible, not to mention you probably wouldn't be happy with the result. Question: I had saline breast implants put in 13 years ago. One has ruptured and the other one I think will rupture soon. I have seen two Doctors about getting them fixed. The first Doctor told me that since I have them over the muscle I absolutely could only get them redone that way. He also told me that the teardrop implants will turn and are no good. The second Doctor recommended silicone high profile teardrop implants, and that he recommends putting them under the muscle. Can I get implants under the muscle if I have them over the muscle now? The vast majoriy of breast augmentations I perform are partially under the muscle and rarely above the muscle. I think this produces a more natural and longer lasting result as well it helps to cover up the top of the implant. What would benefit you depends on your breast size, shape, tissue thickness, etc. Also important is the size of your current implants. Do I understand you to say that one of your implants has deflated? Depending on these factors, it may be possible to place new implants partially under the muscle (site change)but it would impossible to say without the benefit of examining you. Unless you have good thickness of the tissues over your current implants, I think placing new implants in the same pocket above the muscle is not what I would recommend. I hope this helps. It is very impotant to not let the cost of a surgery be the deciding factor in where you have surgery. You need to be knowledgable about the training of the surgeon, are they board certified by the American Board of Plastic Surgery, do they perform surgery in an accredited surgical center, is their anesthesiologist board-certified, etc. I cannot count the number of times a patient has come in to see me for an evaluation for a revision procedure and they tell me they didn't check all of these things out before having surgery. To change from a larger implant to a smaller one, especially after 5 years, often times means that you may have to have a lift of some kind (as you mentioned) due to skin and muscle relaxation. Pam, It is not uncommon to lose breast volume with pregnancy and subsequent breast-feeding. I prefer to place the incision under the breast in the inframammary fold and the implant partially under the muscle. This serves to minimize any disruption of the nipple and breast tissue. This, anatomically, should not decrease the ability to breast feed but no one can guarantee that you could with your next pregnancy. There is currently no recommendation for how often a breast implant needs to be replaced. It depends on what the fill material is of the implant. A saline implant should be changed if it deflates, the patient wants to change sizes or if there is a capsular contracture. If you have the implants for 10 years and they are soft, there is no need to perform any surgery. You do need to continue self breast exam and mammography in the appropriate settings. The normal recovery time is varied from patient to patient. I want you moving about the evening of surgery and then slowly increase your level of activity. I ask you not to lift anything greater than 10 pounds for 6 weeks after surgery. The operative time for a standard breast augmentation is typically 45 minutes. It is important to follow up with your initial plastic surgeon if that is possible. If you have moved or for some reason unable to see them, I would be happy to see you to evaluate how best to help you obtain symmetry. Although thankfully a rare complication, exposure of an implant can occur typically necessitating the need for it to be removed for at least a 3-6 month period of time. This allows for your tissues heal heal and in doing so, become softer. If you were treated for an infection, it is very important to not undergo re-implantation for at least 3-6 months after all of your wounds have healed. The specific procedure that would benefit you will depend upon the degree of breast sagginess or "ptosis" that you have of your breasts with your existing implants. By and large it does not sound as though you need any alteration of your implants, the pocket of the implant or the chest muscle. However, I will be able to tell you with more precision at the time we meet. In terms of recovery, it depends upon the amount of surgery you will need. It is typically 7-10 days of down time but you are not healed enough to get back to any exercise regimine for at least 4-6 weeks and that is a gradual process which is highly individualized.

The incision used to place a breast implant varies from surgeon to surgeon. The two most common incisions are under the breast (inframammary) and through the nipple (peri-areolar). Less commonly is the armpit incision (transaxillary) route used. I do not advocate the belly button (TUBA-transumbilical) approach because it is frought with problems such as malposition of the implant and increased risk of bleeding or bruising. I caution you on picking a surgeon based upon the incision they use. Not all doctor's advertising that they perform breast augmentation are plastic surgeons so you should do your homework first.
If you look at the before and after photos on my website, you will note that more than 95% of them are placed through the inframammary incision. You are smart to think about this important aspect of breast augmentation. I ask my patients not to lift anything greater than 10 pounds for approximately 6 weeks after surgery, especially if your implants are placed partially under the muscle. The risks of going back to dancing too soon are that it could allow your implants to settle too low due to minimal support, incisional healing problems and an overall less than great result. To answer your question, I think you should avoid dancing, without a bra or support, for at least 4 weeks as long as your healing is on track. You will get different answers from different surgeons but keep in mind that you want your end result to be as good as possible for as long as possible. I do remove silicone implants. Implants placed 40 years ago most certainly should be examined by mammogram/MRI and physical exam. I'm not sure what you mean when you say they are collapsed but it sounds as though someone has told you they were ruptured or leaking.
I operate only in fully accredited same day surgery centers. As far as my qualifications, I am board-certified in plastic surgery.
